裏ハムラ法
裏ハムラ法（経結膜眼窩脂肪移行術）とは、下まぶたの裏側（結膜側）からアプローチし、目の下の膨らみの原因である眼窩脂肪を移動・再配置することで、目の下のたるみやクマを根本から改善する手術です。
顔の皮膚表面を切開しないため傷跡が残らず、ダウンタイムも比較的短いことから、近年特に注目されている術式です。院長自身が様々な術式を研究・検討した結果、「目の下のたるみ・クマに対して最も理にかなった手術」と確信し、自ら受けた手術でもあります。

目の下のたるみやクマは、「脂肪の膨らみ」と「その下の凹み」が隣り合うことで、境界に強い影ができることが主な原因です。
この凹みを生み出しているのが、ORL（Orbicularis Retaining Ligament：眼輪筋支持靭帯）という、骨と皮膚を強く結びつけている靭帯です。ORLが皮膚を骨に向かって引き込むことで溝（凹み）が生まれ、その上に眼窩脂肪の膨らみが重なることで、影（黒クマ）として目立ちます。
単に脂肪を抜く「脱脂」だけでは、膨らみは減っても凹みが残るため、影が消えきらないことがあります。裏ハムラ法は、この凹みの原因であるORLを解除した上で脂肪を再配置するため、より根本的な改善が可能です。

理由1：皮膚表面に傷跡が残らない
裏ハムラ法は、下まぶたの裏側（結膜という粘膜）をわずかに切開して行います。メスを入れるのはまぶたの内側だけですので、皮膚の表面には傷跡が一切残りません。
また、粘膜の切開部は自然に塞がるため、後日まぶたの表面から抜糸をする必要もありません。まぶたの表側を大きく切開する術式と比べて皮膚へのダメージが少ないため、腫れや内出血が長引きにくく、周囲に気づかれることなく治療を受けたい方に適したアプローチです。
理由2：靭帯（ORL）を解除し、段差をなくす
裏ハムラ法では、凹みの原因となっているORL（眼輪筋支持靭帯）を丁寧に剥がして骨から解放します。引き込まれていた凹みのロックが解除されると、そこに眼窩脂肪を移動・再配置し、滑らかに固定します。
自分自身の組織だけを使って膨らみと凹みの「段差」をフラットに整えることができるため、不自然に窪むことなく、若々しく滑らかな目元に仕上がります。
理由3：脂肪を再配置するから再発しにくい
裏ハムラ法は、脂肪を「取り除く」のではなく「活かす」手術です。膨らんでいる部分の脂肪を、すぐ下の凹んでいる部分へ移動させて固定するため、脂肪が同じ場所に再び突き出てくるリスクが非常に低くなります。
また、加齢とともに目の周りの骨や脂肪は少しずつ萎縮し、目元が窪みやすくなります。若い頃に脂肪を多く抜きすぎると、年齢を重ねたときに目元が深く窪んでしまう原因になることがあります。裏ハムラ法は脂肪のボリュームを温存したまま位置を変えるだけですので、将来的な窪みリスクを抑え、自然なエイジングを叶えます。

初診カウンセリング
目の下の状態を確認し、治療のシミュレーションと写真撮影を行います。手術を安全に行うための血液検査（採血）も実施します。常用薬などがある方はおくすり手帳をご持参下さい。
採血から約2週間後
採血結果に問題がなければ、手術の日程を決定します。同意書の内容を確認いただき、ご不明点があればこの時にお答えします。
手術当日
鎮静（静脈麻酔）と局所麻酔の併用で行う日帰り手術です。当日はご自身での運転できません。手術時間は症状や程度によって異なりますが、1時間から2時間で終了します。
翌日以降
洗顔・洗髪・シャワーは翌日から可能です。飲酒・入浴・激しい運動は数日後から再開できます。腫れや内出血は個人差がありますが、徐々に改善していきます。
経過観察
術後は状態に応じて来院していただき、経過を確認します。

Q. 手術後、どのくらいで腫れが引きますか？
個人差はありますが、大きな腫れは1〜2週間程度で落ち着いてくる方が多いです。完全に馴染むまでには数ヶ月かかる場合がありま す。

Q. 効果はどのくらい持続しますか？
脂肪を移動・固定する手術のため、同じ部位への脂肪の再突出が起きにくく、長期的な効果が期待できます。ただし加齢による変化は継続しますので、個人差があります。
